351,195 cases of Lyme disease have been reported to the Centers for Disease Control, 1980 - 2009 (06/27//09). There have been 4,433 cases of Lyme Disease Reported to the Centers for Disease Control for the year 2009 (06/27/09). Remember it is conservatively estimated that only one in 10 cases of Lyme disease is actually reported to the CDC - so there may have been at least as many as 3.51 million cases of Lyme disease since 1980 and 44,330 cases in 2009.

Available here, a sample letter for patients to submit to their doctor to try and protect their medical records from being seized by a government agency outside of a subpoena. This may prevent states from conducting a "fishing expedition" by requesting medical records. The licensing boards may still obtain records, but they will need to have enough data to convince a judge to subpoena the records.
